Mounjaro and Zepbound are both tirzepatide, both from Eli Lilly, and both run 2.5, 5, 7.5, 10, 12.5 and 15 mg once weekly. The dose ranges are identical, which makes this a cleaner version of the Ozempic and Wegovy split: here the only difference is what each brand is licensed to treat.

The licences

Mounjaro is licensed as an adjunct to diet and exercise to improve glycaemic control in adults and children aged 10 and over with type 2 diabetes. That is its whole label.

Zepbound is licensed for long-term weight reduction in adults with obesity, or adults with overweight plus one weight-related condition, and separately to treat moderate to severe obstructive sleep apnoea in adults with obesity.

That sleep apnoea indication is the only one of its kind among the nine approved brands, and it is a genuine difference rather than a labelling technicality.

What tirzepatide does that the semaglutide brands do not

Tirzepatide is an agonist at two receptors, GIP and GLP-1. The semaglutide and liraglutide brands act at GLP-1 only. Whether the second receptor is why tirzepatide outperforms semaglutide on weight in a head-to-head, or whether the doses simply are not equivalent, is not settled by the trial that measured the difference.

Boxed warning

Both carry the same boxed warning for thyroid C-cell tumours. It is a class warning across every GLP-1 brand in this comparison except one, and it is the reason a personal or family history of medullary thyroid carcinoma or MEN 2 is a contraindication.
